Unexpected Shutdown Issues on iPhone 6s
As

Apple European USB Power Adapter Exchange Program

Apple has determined that in certain rare cases the USB power adapter
European 5 W of some iPhones could overheat and cause problems
security issues. This questionable adapter was supplied with the models
of iPhone 3GS, iPhone 4 and iPhone 4s shipped between
October 2009 and September 2012. It was also sold separately
as an accessory. If the "CE" behind the adapter is written in letter
full and dark gray, then it is an adapter to replace.
